Making a referral for individualised support at Te Waka

Making a referral is easy. However, due to high demand and not enough provision to meet it, there are often wait times. We will endeavour to get back to the referrer with the estimated time of wait, as well as when we start the service.

There are two referral pathway options: 

1) Sorry. This pathway is not currently available for Te Waka Youth Services. You may use it to result in referral to another counsellor through Your Choice, or complete the referral process #2 below, to access Te Waka Youth Services. 
If the young pers
on is aged 12 to 24 years of age, living within the Rodney boundary and presenting with Mild to Moderate mental health concerns. You may wish to make a YOUR CHOICE Referral. This then goes through a brief triage at the Waitemata DHB, before coming back to Te Waka with approval for 4 funded sessions. Be sure when emailing to youthhub@healthwest.co.nz that you CC in Tewakayouth@gmail.com, so that if it is not approved we can still pick it up under our general system.
